Rescue cervical cerclage may be performed when a woman is admitted with silent cervical dilatation and bulging of the membranes into the vagina but without the onset of uterine contractions. Characteristically such women present with slight vaginal bleeding, a watery vaginal discharge, or vague pelvic or vaginal pain. One small prospective-non-randomized study has suggested that rescue cervical cerclage improves birth weight and is not associated with a significant increase in the frequency of chorionamnionitis, maternal morbidity or perinatal mortality. The median pregnancy prolongation following emergency cervical cerclage is approximately 7 weeks. Whether antibiotics are beneficial in such cases has not been established.
