Ephelides are physiologic areas of enhanced melanin production and are a response to a natural stimulus. They are dominantly inherited and will recur with solar exposure. They can be lightened with various bleaching preparations, but this is usually successful only when combined with a monastic indoor existence. Provide these fair-skinned, skin cancer prone patients with support, a kindly explanation, and a discussion of proper sun avoidance and protection with a high SPF Parsol® containing sunscreen. Although there are methods for removing ephelides, the risks outweigh the potential benefits.
